Consular Services

The consulate is open from Monday to Friday from 0900h to 1500 h. Appointments beyond these hours can be made via the website by email. For emergencies, you may reach out to the consulate at the number 422263 or 597-879 8669 or 597-851 9161.

Filipino citizens in Suriname are offered the following consular services:

1. ATN
a. Assist Filipinos, regardless if resident or transient
b. ATN-related expenses shall be requested from the Embassy
c. Visits in prison and welfare assistance- list of items with total price to be sent to the Embassy for request of funding to Manila. Medicines or hospitalization, if urgent, can be for reimbursement.

2. Consular documentation
a. Acknowledgment and notarization of legal affidavits and certificates, including work contracts
b. Acceptance of documents for civil registration

Report of Marriage

Documents issued by a foreign country (not Philippines) should be translated by a sworn translator (if not in English) and Apostilled.

 

If the Report is filed a year after the marriage, an Apostilled Declaration or Affidavit stating reasons for the delayed registration is required.

 

1.   Duly-accomplished Report of Marriage Form (1 original, 3 photocopies)

2.   Widowed Filipino applicant

a.    DFA Apostilled Death Certificate of previous partner

Report of Birth

A.   For children whose parents are married

1.   Duly-accomplished Report of Birth Form (1 original, 3 photocopies)

2.   Certificate of Birth issued by Civil Registrar (1 original, 3 photocopies)

3.   For parents who were married in the Philippines, PSA-issued Marriage Certificate

4.   Passport data page of both parents (4 photocopies)

5.   For Filipino parents, RNE/Protocolo or MKV (4 photocopies)

6.   If child was born prior to the marriage of parents, an Affidavit of Legitimation is required

7.   For parents who were married outside the Philippines:

a.    If birth of child occurs within one year from date of marriage, Report of Marriage from Philippine Embassy/Consulate (1 original, 3 photocopies)

b.   If birth of child occurs after one year from date of marriage, PSA-issued Marriage Certificate (1 original, 3 photocopies)

 

B.   For children whose parents are not married and will use the father’s surname

1.  Duly-accomplished Report of Birth Form (1 original, 3 photocopies)

2.   Certificate of Birth issued by Civil Registrar (1 original, 3 photocopies)

3.   Affidavit of Acknowledgment/ Admission of Paternity (4 sets)

4.   Affidavit to Use the Surname of the Father (4 sets)

C.   For children whose parents are not married and will use the mother’s surname

Report of Death

If the Report is filed a year after the Filipino’s death, an Apostilled Declaration or Affidavit stating reasons for the delayed registration is required.

 

1.   Duly-accomplished Report of Death Form (1 original, 3 photocopies)

2.   Passport data page of the deceased Filipino (4 photocopies) or PSA-issued Birth Certificate

3.   Death Certificate (1 original, 3 photocopies)

4.   Certificate issued by Mortuary/Funeral Director (1 original, 3 photocopies)

5.   Certificate of Non-Contagious Disease (1 original, 3 photocopies)

6.   Transit Permit (1 original, 3 photocopies)

7.   Cremation certificate, if applicable

8.   Information of the Consignee

a.       Name and address of consignee in the Philippines

b.      Name and address of funeral home in the Philippines

 

Filling-in the Form:

1.   Item #15: Make sure the immediate cause of death is identified, otherwise the form will not be accepted for registration.

2.   Items #20-21: The flight no. and date of shipment should not be left blank.

3.   Item # 26: Make sure the informant and their relationship to the deceased is identified.

NBI Clearance

1.   Letter requesting for NBI Form

2.   If through mail, vale postal (Brazil) or for pick-up by applicants preferred courier

3.   2×2 photo with white background, taken within the last three months

 

It is preferred that the fingerprinting be done in the presence of the Consul however, impressions can also be taken at the nearest police agency.

 

The person who took the fingerprint impression will have to sign and state his/her official designation on the designated space in the Form.

 

NBI Form No.5 is free.

Travel Document

Reminder for Applicants

  1. Arrange for return mailing of original documents

    1. PSA-issued marriage certificate must be presented prior to the use of the husband’s surname in the passport

    1. PSA-issued birth certificate must be presented prior to the child’s first-time passport application
  1. Report of Death
    1. Return mailing- vale postal or pick-up by preferred courier
  1. NBI Form- Applicants requesting for NBI Form are advised to execute as well a Special Power of Attorney for the person they will be authorizing in the Philippines to process their application with the NBI Main Office.

The Philippine Embassy in Brasilia, Brazil, conducts regular outreach missions to bring various consular services to Filipinos residing in Suriname. The schedule of the Outreach Missions can be inquired from the consulate.

In order to streamline consular outreach operations and to ensure an efficient and comfortable process for everyone, passport applications will be on a per-appointment basis.
